All new patients of Dr. Frank Cherpack are required to fill out the Patient Information Form and Medical History Form.
Please print out and complete the following forms. This will save you and staff time when you get to Dr. Cherpack’s office and allow you more time with the foot doctor.

Cash, Check, or major Credit Card. Deductible amount, co-insurance, or any balance not paid by your insurance is due at time of service.
Meet Roxy, our friendly and gentle office Mascot. Roxy is often found here at the office when she feels like coming to work. When she is here, she will greet you at the door and maybe ask you for a belly rub.
